The 1% cream contains the active ingredient clotrimazole at a concentration of 10 mg/g and auxiliary components: cetostearyl and benzyl alcohols, polysorbate 60, synthetic spermaceti, octyldodecanol, sorbitan stearate, water.
The 1% ointment contains the active ingredient clotrimazole at a concentration of 10 mg/g and auxiliary components: polyethylene oxides 400 and 1500, propylene glycol, methylparaben, nipagin.
The 1% solution contains the active ingredient clotrimazole at a concentration of 10 mg/ml (in terms of dry matter) and auxiliary components: polyethylene glycol 400, propylene glycol, ethanol 96%.
1 suppository (candle) contains 100 mg of the active ingredient clotrimazole and semi-synthetic glycerides in the volume required to obtain a suppository weighing 2 g.
The tablets contain 100, 200 or 500 mg of the active ingredient clotrimazole and auxiliary components: lactose in the form of monohydrate, potato starch, adipic acid, sodium bicarbonate, silicon dioxide in colloidal form, sodium lauryl sulfate.

Pharmacodynamics and pharmacokinetics
The active substance of the drug is an imidazole derivative. It is effective against pathogenic fungi, including the following microorganisms:
- dermatophytes;
- dimorphic fungi;
- yeast-like fungi.
The mechanism of action is to block the synthesis of nucleic acids, proteins and ergosterol. These substances are necessary for the construction of microorganism membranes. Due to the suppression of their synthesis, fungal cells are damaged and it dies. A low concentration of the drug has a fungistatic effect. It consists in suppressing the growth of fungus. At a dose above 20 μg/ml, the drug exhibits a fungicidal effect, i.e. destroying microorganism.
When used topically, the active substance is absorbed by the epidermis. The highest level of concentration is observed in the reticuloderm. Clotrimazole also penetrates the keratin of nails. The drug practically does not penetrate into the systemic circulation. In the liver it is metabolized to inactive substances, which are then excreted in the urine. When used intravaginally, i.e. when introduced into the vagina, 5-10% of the active component is absorbed. The substance is also metabolized in the liver.
Indications for use
Clotrimazole for men against thrush is used to treat certain diseases. Common indications are skin damage by fungal microorganisms such as yeast, mold, and dermatophytes. The drug also helps in cases of lichen and erythrasma. Other indications for use:
- nail fungus (onychomycosis);
- trichophytosis;
- candidiasis;
- dermatomycosis;
- fungal erosion;
- athlete's foot;
- candidiasis;
- genital superinfections.

In vitro, it exhibits high activity against the most pathogenic fungi that can cause serious infectious complications, namely:
- dermatophytes (Microsporum sp., Epidermophyton floccosum, Trichophyton sp.);
- yeast-like fungi (Cryptococcus neoformans, Candida sp.);
- dimorphic fungi (Histoplasma capsulatum, Paracoccidicides brasiliensis, Coccidiodes immitis).
The mechanism of action of clotrimazole is based on its ability to block the synthesis of proteins, nucleic acids and ergosterol, which is necessary for the construction of cell membranes of fungi. As a result, the cell membranes of the fungi are damaged and the cells die.
At low concentrations, clotrimazole has a fungistatic effect. If the dose exceeds 20 mcg/ml, the drug has a fungicidal effect, and not only on proliferating cells.
When used in high doses, it helps to increase the concentration of hydrogen peroxide in the fungal cell to a toxic value and, as a result, destroys the fungal cells.
When applied topically, the drug is absorbed by the epidermis. The highest concentration of the substance is determined in the reticuloderm. It is also able to penetrate the keratin of nails.
Clotrimazole enters the systemic circulation in very small quantities. Metabolized in the liver to inactive substances that are excreted from the body in feces and urine.
When administered intravaginally, 5 to 10% of clotrimazole is absorbed. The absorbed substance is metabolized in the liver. Metabolic products are excreted in bile.

Pharmacokinetics
It is poorly absorbed through the skin and mucous membranes and has virtually no systemic effect. Accumulates in the stratum corneum of the epidermis, penetrates into the keratin of the nails. The concentration in the deep layers of the epidermis is higher than the minimum inhibitory concentration for dermatophytes.
Pharmacodynamics
An antifungal agent from the group of imidazole derivatives for external use. Clotrimazole reduces the synthesis of ergosterol, which is an integral part of the cell membrane of the microbial wall, and leads to a change in its structure and properties. In small concentrations it acts fungistatically, and in large concentrations (more than 20 μg/ml) it is fungicidal, and not only on proliferating cells. At fungicidal concentrations, it interacts with mitochondrial and peroxidase enzymes, resulting in an increase in the concentration of hydrogen peroxide to a toxic level, which also contributes to the destruction of fungal cells. Active against pathogenic dermatophytes (Trichophyton rubrum. Trichophyton mentagrophytes, Epidermophyton floccosum, Microsporum canis); yeast and mold fungi (Candida spp., Torulopsis glabrata, Rhodotorula spp., Pityrosporum orbiculare).
Indications for use
Fungal diseases of the skin, including skin folds and feet, caused by dermatophytes, yeast (Candida spp.), mold and other fungi and pathogens sensitive to clotrimazole: dermatomycosis, dermatophytosis, trichophytosis, epidermophytosis, microsporia, candidiasis, interdigital fungal erosion, fungal paronychia.
Mycoses complicated by secondary pyoderma, as part of complex therapy.
Lichen versicolor, erythrasma.
Contraindications for use
Hypersensitivity to clotrimazole or excipients, pregnancy (first trimester), children under 2 years of age.
With caution: lactation period, pregnancy (II and III trimester).
Use during pregnancy and children
The use of the drug in the first trimester of pregnancy is contraindicated. Clinical and experimental studies have not found that the use of the drug during pregnancy (II and III trimester) or during breastfeeding has a negative effect on the health of the woman or fetus (child). Application of the drug directly to the lactating mammary gland is contraindicated.
Side effects

Dosage
Externally.
For adults, in the absence of other prescriptions, it is recommended to apply the ointment 2-3 times a day in a thin layer to previously cleansed and dry affected areas of the skin and rub in gently. A single dose for a palm-sized surface area is a column of ointment 5 mm long. Regular use of the ointment is important for successful treatment. The duration of therapy is individual and depends on the severity and location of the disease. To achieve complete recovery, you should not stop treatment with the ointment immediately after the disappearance of acute symptoms or subjective complaints. The duration of therapy should average about 4 weeks.
Lichen versicolor is usually cured within 1-3 weeks, and erythrasma within 2-4 weeks.
For fungal diseases of the skin of the feet, it is recommended to continue therapy for about 2 weeks after the symptoms of the disease have subsided.
In children over 2 years of age, the drug is used in the doses indicated above.
Overdose
Low systemic absorption of the active component of the drug when applied externally makes overdose almost impossible.
Precautionary measures
Contact with the mucous membrane of the eyes should be avoided.
In patients with liver failure, the functional state of the liver should be periodically monitored.
If signs of hypersensitivity or irritation appear, treatment is stopped. If there is no effect within 4 weeks, the diagnosis should be confirmed. In children, avoid application to large surfaces.

Candles - what are they for?
The indication for the use of vaginal suppositories is vaginal candidiasis and genital superinfections caused by microorganisms sensitive to clotrimazole. The drug is prescribed to women and, after the onset of sexual activity, to teenage girls.
What are Clotrimazole tablets for?
Clotrimazole tablets are used to treat blastomycotic infections of the vulva and vagina. In particular, the drug is prescribed for vaginitis caused by fungi of the genus Candida (including diseases caused by strains resistant to nystatin), caused by other pathogens sensitive to clotrimazole, superinfections of the genital organs, for sanitization of the genital tract before delivery.
Directions for use and dosage
Since suppositories and tablets are intended for intravaginal administration, they are not prescribed to men. To treat representatives of the stronger sex, ointment, gel, cream and solution are used. The duration of therapy is determined by the specific disease. It consists of external use of a cream, ointment, gel or solution. In men, it is difficult to treat thrush, because it can be asymptomatic, affecting not only the genitals, but also other places, for example, the mouth.
Clotrimazole ointment for men
Before applying the ointment to areas affected by fungus, they must be washed with soapy water and dried thoroughly. You need to lubricate the skin with the preparation, covering a small healthy area nearby. Apply the ointment in a thin layer up to 3 times a day, lightly rubbing until completely absorbed. To treat an area of skin approximately the size of the palm of your hand, a strip of the drug 5 mm long is required. For male thrush, in case of damage to the glans, you need to apply the ointment to the foreskin of the penis.
Treatment continues until symptoms disappear and for another 2 weeks after their relief for prevention. Course of therapy for certain diseases:

Gel
Instructions for using the gel are similar to the principle of applying the ointment. The drug is distributed over clean, dried skin, affecting small healthy areas nearby. The gel is rubbed in with light massaging movements up to 2-3 times every day. This form of the drug is distinguished by the fact that it is well distributed over the skin and is quickly absorbed. It is convenient to use on hard-to-reach areas of the body.
Clotrimazole cream for men
Candidiasis and other fungal infections can be treated with cream. They lubricate the affected areas 2 or 3 times a day, lightly rubbing. The skin must first be washed and dried. Clotrimazole cream for men is used in the same way as the ointment. This applies to the treatment of candidal balanitis and balanoposthitis. The course of treatment in this case is 1-2 weeks. Therapy for dermatomycosis lasts 3-4 weeks. To exclude relapses of the disease, it is necessary to continue treatment for another 2 weeks after the symptoms of candidiasis disappear.
Clotrimazole solution
Before the application procedure, the skin must be washed with soapy water. This is especially true for areas between the fingers. After washing, the skin is dried and only then the solution is applied. They treat the lesions 2-3 times every day. The amount of solution should be such that the entire surface of the damaged skin can be moistened. If your feet are affected by mycosis, then you need to use the solution after each wash. Other features of the use of this form of the drug:
- This form is preferable in the case of onychomycosis, i.e. nail fungus.
- If the oral cavity is affected, it is treated with a cotton swab or swab up to 4 times a day. After the procedure, you need to refrain from drinking drinks and food for about an hour.
- Clotrimazole drops can be used to treat men's ears, into which cotton swabs soaked in the drug are inserted. They are placed for 5-10 minutes 3-4 times daily.

Side effects
Allergic reactions.

Local use for urogenital mycoses may be accompanied by burning, itching, swelling and hyperemia of the mucous membrane, intercurrent cystitis , the appearance of vaginal discharge, increased urination, pain during sexual intercourse.
When applied topically for mycoses of the oral cavity, redness of the mucous membrane, tingling and burning sensation at the site of application of the drug, and irritation may be observed.

Clotrimazole ointment, instructions for use
Before applying Clotrimazole ointment (Acri or other manufacturers), the affected areas of the body are first washed with soapy water and dried thoroughly.
The drug is applied to the affected areas (covering small areas of healthy skin around the lesion) in a thin layer from 1 to 3 r./day. and rub lightly.
For 1 treatment of an area the size of a palm, a 5 mm strip of ointment is sufficient.
Regardless of what pathogen causes the disease, treatment after relief of subjective symptoms is recommended to continue for another 2 weeks.
For dermatomycosis, the course of treatment lasts from 3 to 4, for erythrasma - from 2 to 4, for lichen versicolor - from 1 to 3 weeks.
If there is no clinical improvement after 4 weeks of regular use of the drug, the patient is sent for a repeat microbiological study and the treatment regimen is revised.
The use of ointment for thrush in women helps block the spread of fungal cells and, when higher doses are prescribed, ensures their complete destruction. For therapeutic purposes, Clotrimazole for thrush is used 2 times a day, rubbing the ointment into the affected areas of the perineum.
Until the drug is absorbed, it is recommended not to wear underwear.
For men, the method of use for thrush and other fungal infections of the genital organs ( candidal balanoposthitis , urogenital candidiasis ) is the same as for women.
The ointment is applied to the foreskin and glans penis and gently rubbed into the skin and mucous membrane. Frequency of applications - 2 times/day.
The drug is poorly absorbed into the skin, and its active substance is characterized by the ability to gradually accumulate in the upper layer of the skin. Thanks to these properties, Clotrimazole ointment is highly effective for thrush in men and women.
Clotrimazole cream, instructions for use
The cream is intended for topical use. For thrush and other fungal diseases, apply it to the affected areas twice or thrice during the day and rub in lightly.
If mycoparasites affect the genitals, to treat thrush, the cream is prescribed simultaneously to both partners. For candidal balanitis or balanoposthitis, clotrimazole cream for men is used in the same way as ointment.
The duration of the course depends on the indications for use and ranges from 1-2 (for candidal balanitis/vulvitis ) to 3-4 weeks (for dermatomycosis ).
To prevent relapse of the disease, it is very important not to stop treatment for another 2 weeks after the subjective symptoms of infection disappear. If there is no improvement after 4 weeks of therapy, the patient should consult a doctor.
Vaginal cream Clotrimazole is administered into the vagina in portions of 5 grams 1 time per day. within 3 days.
Instructions for Clotrimazole solution
Before applying Clotrimazole, the skin should be washed with warm soapy water (especially the areas between the fingers) and dried well. The affected areas are treated 2-3 times a day. The amount of solution should be sufficient for uniform application to the entire affected surface.
If the skin of the feet is affected, liquid Clotrimazole solution must be applied after each wash.
The solution is the most preferred dosage form for onychomycosis . This is due to the fact that it has the best penetrating properties.
For fungal infections of the oral cavity, the affected areas of the mucous membrane are treated with a cotton swab or swab, onto which drops are first applied (10 to 20 drops are enough to treat each ear). Frequency of procedures - 3-4 rubles/day.
After applying Clotrimazole, you should refrain from eating and drinking for an hour.
Clotrimazole drops are also effective against onomycosis caused by yeast fungi. Drops are administered into the ear using turundas: a twisted cotton ball is moistened in the solution and 3-4 times a day. Place it in the ear canal for 5-10 minutes. Treatment lasts from 3 to 4 weeks.
Instructions for Clotrimazole gel
The method of using the gel is similar to the method of using the ointment. The product is applied to the epidermis affected by mycoparasites 2-3 times a day. and rub in with light movements.
The peculiarity of this dosage form is that the gel is well distributed on the skin and is also well absorbed into it. This allows the drug to be used for treating hard-to-reach areas of the body.
The gel is used intravaginally in the same way as clotrimazole cream: 1 r./day. 5 grams each. The course of treatment is 3 days.
Clotrimazole vaginal tablets, instructions for use
In the absence of other prescriptions, Clotrimazole Acri tablets and synonymous drugs for thrush and other fungal infections of the genital organs are used:
- for 3 days, 1 tablet 2 times a day;
- for 6 days, 1 tablet 1 time per day.
How to administer Clotrimazole vaginal tablets? Before administration, the tablet is removed from the package and moistened with chilled boiled water. The drug should be administered in the “lying on your back” position with your knees bent.
In order for the tablet to completely dissolve and clotrimazole to work, a moist environment is necessary (otherwise, undissolved particles may spill out). Therefore, the tablet is inserted into the vagina as deeply as possible. It is optimal to carry out the procedure before bedtime.
Treatment must be stopped before menstruation begins. Menstrual flow will wash away the drug and it will not have the desired effect. During treatment with vaginal tablets, you should not use tampons, spermicides, intravaginal douching or any other vaginal products.
To sanitize the genital tract before delivery, a woman is given a single dose of 200 mg of clotrimazole.
During pregnancy, the drug is contraindicated in the 1st trimester. At later stages, clotrimazole does not have an adverse effect on the development of the fetus, however, the use of the applicator may be associated with certain risks.
Instructions for Clotrimazole suppositories
Suppositories for thrush are used intravaginally. The drug should be injected deep into the vagina, one dose daily for 6 days. A repeat course is possible after consulting a doctor.
Clotrimazole suppositories for thrush and other urogenital mycoses are not used during menstrual bleeding. Treatment must be completed before it begins.

How to take Clotrimazole
Recommended dose:
Clotrimazole cream is injected as deep as possible into the vagina once a day (preferably before bedtime) for 3 days without a break. A sufficient course of treatment with the cream is 3 intravaginal applications, according to the procedure described below. Additionally, a cream may be needed to relieve the external itching and burning sometimes associated with a vaginal yeast infection. Clotrimazole is for vaginal use only and should never be taken orally. The treatment course should not be carried out during the menstrual cycle. Treatment should be completed before the onset of menstruation. Although it is possible that sexual relations may occur during treatment with Clotrimazole, it is preferable for couples to wait until the end of the treatment course to prevent infecting your partner.
Applicator filling:
Remove the cap from the Clotrimazole tube, press and pierce the aluminum foil with the tube cap. To refill the applicator, screw the open end of the applicator barrel onto the threaded neck of the tube. Carefully begin squeezing the cream out of the tube. After this, the upward movement of the piston draws the required amount of cream into the applicator cylinder. As the applicator fills, the piston moves outward. When the plunger reaches its stop and stops, the proper amount of Clotrimazole has already been filled into the applicator and the applicator can be unscrewed from the tip of the tube. Close the cap and reserve the tube for the next use.
Insertion of the cream: Clotrimazole cream is inserted into the vagina in the same way as a tampon, standing, crouching or lying on your back, in a position convenient for you. Insert the filled applicator into the vagina. The depth of insertion depends on how comfortable you feel during insertion. Hold the applicator cylinder steady and insert the cream into the vagina, gently pressing the applicator piston until it stops. Inject the cream as deep into the vagina as possible, which will ensure the maximum effect of the drug. Remove the applicator.

External use of the cream:
A small amount of Clotrimazole cream may be applied to the external opening of the vagina to provide additional relief from external symptoms. Squeeze a small amount of cream onto your finger and gently spread over the irritated vaginal area. The cream should be used once or twice a day and only during the period when external symptoms are present, no more than 7 days.
